Based on Articles 15, 16 and others of the Federal Law for the Protection of Personal Information in the hands of Third Parties (LFPD, Spanish initials) and its regulations, we hereby present our Privacy Policy:

The company that is in possession of your personal information is SAKAR DENTAL S. C. whose object is the provision of dental healthcare and hygiene, specializing in restorative and cosmetic dentistry (hereafter “the Company”) whose address is at Presa Sanalona 8, Colonia Irrigacion, CDMX, 11500, Mexico, Phone: (55) 5557-1138

II. Means whereby the personal information was obtained.

For the purposes mentioned in this Privacy Notification, the Company may obtain your personal details through the present form when you come for your first appointment.

III. Required information

The information required is the following identification and contact information; invoicing details and dental records (hereafter referred to jointly as “Personal Information”).

IV. The reason why personal information is required

The personal information mentioned in III above will be used by the Company for the following:

A. Legal purposes

• A.1. For identification and administrative purposes so that patients can be provided with dental services.
• A.2. For the purpose of creating and updating hard copy and digital files and databases of all our patients so that the corresponding dental services can be provided.
• A.3. For the creation of fee receipts and invoices.

B. Non-legal purposes

• B.1. Sending advertising and promotional material on our special services.

The purposes mentioned in B. above are not required for purposes of our legal relationship with you, but they serve to keep you informed of our services and also to monitor and improve their quality. You have the right to withdraw your consent that your personal information be used by the Company for this purpose, in accordance with the procedure mentioned in Point VI of the present Privacy Notification.

V. Options for limiting the use or disclosure of your personal information

VI. Request to exercise your arco rights

In accordance with the LFPD and its regulations, the Company guarantees the privacy of your personal information and will use, store, distribute, and physically and electronically keep said information solely for the established purposes. The Company has designated Doctor Marisela Gutíerrez R. as the Officer in charge of Personal Information Protection, to whom you may address any requests to exercise your ARCO (access, rectification, cancellation, and opposition) rights or any other doubts or concerns you may have regarding the use of your personal information. All your personal information is treated in accordance with the applicable legislation in force in Mexico and we reiterate that you have the right at all times to cancel or oppose the use we make of your personal information, including the revocation of your consent to use said information. You may exercise this right simply by sending an email with your request to Doctor Marisol Gutiérrez Ramírez at: privacidad@sakardental.com .

Your request must contain at least the following information: (i) Full name and email address or street address, so we can respond to your request; (ii) proof of identification documents or, if applicable, that of your legal representative; (iii) a clear description of the personal details over which you wish to exercise the above-mentioned rights; (iv) any other document or relevant information that will facilitate the location of the Personal Information. Your request will be attended to within the time period established by law, and we will inform you of the result of your request via the email address you provided to us for this purpose.

II. Changes to the privacy notification

This Privacy Notification may be amended or changed at any time by the Company; any changes or modifications are done automatically and you will be informed of any such change by any of the following means:
